The GS-6 Form (Program of Study Form) provides a record of the program of study the graduate student will complete to earn the degree and identifies the committee members (if applicable). The GS-6 Form is due before the end of the second term of enrollment after being admitted as a graduate student (including summer) or after completing 12 semester credits, whichever comes first. This applies to part-time and full-time students. All necessary signatures must be obtained prior to the form being submitted to the Graduate School. You must complete the GS-6 form electronically by using RAMweb. Instructions can be found on the Graduate School's web site (.pdf). Additional information for Master of Accountancy graduate students can be found on the Information for New Students page.
INSTRUCTIONS FOR GS6
- The form can be found on the Graduate School website.
- Fill in name, ID, phone, email, and semester admitted. Program is ACCY-MACC, degree is Master of Accountancy. Leave the area of study line blank.
- The courses to be listed are only those ten courses you and your advisor have agreed will constitute your program of study. They should be listed under either the “prior” or “after” admission part as appropriate. The total credits should add to 30. Do not list prerequisites.
- Our program does not require a faculty committee, or have non-course requirements, so ignore those parts. However, the name of the advisor must be filled in. Dr. Samelson is advisor to all MAcc students.
- Sign as indicated, and bring to Dr. Samelson for review and further processing.
