The GS-6 Form (Program of Study Form) provides a record of the program of
study the graduate student will complete to earn the degree and
identifies the committee members (if applicable). The GS-6 Form is
due before the end of the second term of enrollment after being admitted
as a graduate student (including summer) or after completing 12 semester
credits, whichever comes first. This applies to part-time and full-time
students. All necessary signatures must be obtained prior to the form
being submitted to the Graduate School. Contact your adviser if
you have questions about this form.
