Please use the following checklist to ensure that your application to the Online MBA Program is complete. All documents are due by the application deadline.
Graduate School Application Form
Complete
Graduate School Application Form online. When completing the form, select “Business Administration MBA – Online Professional” as the “Intended Major.” Also, select fall or spring semester for “Term of Entry.” We do not offer summer admission.
Fee
Pay $50 application fee with a major credit card upon completion of your online application. Your application cannot be processed until the application fee is received.
Statement of Purpose
Include a letter stating your purpose for pursuing a graduate degree and the value you feel you bring to the program. This may be attached to the online Graduate School Application Form or sent to the College of Business as
submission address.
Résumé
Submit current résumé by attaching it to the Graduate School Application Form or by sending it to the College of Business at the
submission address. It is important that your résumé show the month and year you started and the month and year that you left each professional position you have held. International students, please also provide the city, state (if applicable), and country for each position held. Additionally, make sure the country is noted for all educational institutions.
Recommendations
Submit three professional recommendations using the College of Business Recommendation Form (
pdf version). These may be completed electronically and emailed or faxed direct from the recommendor to the College of Business, or mailed at
submission address.
Transcripts
Applicants must present a baccalaureate degree from a regionally accredited four-year college or university. Submit one official transcript from each institution you have attended. These may be sent directly to the College of Business or issued to you in a sealed envelope and then sent to the College of Business at the
submission address. Do not open the envelope in which they arrive before submission. We will accept an official transcript via email directly from the institution; however, we cannot accept faxes. International applicants must submit one set of official transcripts and a degree statement with a certified translation into English from each college or university attended. Documents must show courses and final examinations taken, marks received for each year, and degree or diploma received. If your G.P.A. is under 3.0, include a letter to the Graduate Admissions Committee explaining the circumstances related to academic performance below the requisite
3.0 cumulative GPA for undergraduate or graduate study.
Test Scores
GMAT/GRE: Request GMAT/GRE scores to be sent directly to the University. Scores must be sent from Pearson VUE (GMAT) or Educational Testing Service (GRE) to Colorado State University. The school code is 4075. To make your request, visit
www.mba.com/mba or
www.gre.org. Test Scores must be no more than five years old. If you
qualify and opt to petition for a GMAT/GRE waiver, submit the GMAT/GRE Waiver Petition Form (
pdf version). One of your three recommendations should be from a current manager or supervisor who can attest to the content and accuracy of your resume.
TOEFL/IELTS (International Applicants): Request TOEFL/IELTS scores to be sent directly to the University. Scores must be sent from Educational Testing Service to Colorado State University. If you previously have taken the TOEFL/IELTS but have not had your test scores reported to Colorado State, you will need to have your test scores sent to the University at school code 4075. www.ielts.org. Test scores must be no more than two years old.

Tuition Reduction Program (Military/Veteran/State or Federal Employees)
Military personnel and veterans: Submit a copy of your military ID, separation papers, or a copy of recent orders to qualify for the Military Tuition Reduction Program. Federal and State (other than Colorado) employees: Submit a copy of a recent pay stub clearly showing the organization with whom you are employed to be eligible for the Tuition Reduction Program. Be sure to mark out the Social Security Number on all documents.
